Do let me know if you do receive the USBs or not. As for the toxicology reports for Abel Alvarado, Yolanda Alvarado, and
Elena Marron they were included in the email sent to you June 22.
I want to confirm that when you requested a toxicology report that you meant the toxicology report for each of the
deceased and not for the driver of the vehicle that hit the vehicle that the Alvarado’s and Marron were traveling in. If
so, you have the reports for each of the deceased and there is nothing for me to upload or request from the Riverside
County Sheriff’s office (Palm Desert Police).

I’ve been checking on it but assumed that delay was due to holiday and possible backlog of documents that our support
staff received over the holiday. I will wait unƟl Monday the 10th, if not, I will assume that it was lost in the mail.
In the meanƟme, if you want to upload the toxicology reports to the Sharepoint link in the email above, I can at least
review those for now. Please let me know once you upload them.

I was wondering if you received the 13 USB that were sent to you regular mail, upon request. AddiƟonally, if so, did one
of the USB have the MART report or toxicology records you requested? The Riverside County Coroner’s report emailed
along with the claims included toxicology reports for Abel Alvarado, Yolanda Alvarado, and Elena Marron.
Let me know if I need to inquire further with the Riverside County Sheriff’s office (Palm Desert Police) and/or how I may
be of further assistance.

Notification:
On Wednesday 12/28/2022, at about 1913 hours, deputies were stopped on northbound Monterey Ave
at the intersection of Fred Waring Dr. Deputies observed a vehicle traveling westbound on Fred Waring
Dr at a high rate of speed, which they estimated was over lOOmph. Deputies attempted to locate the

vehicle when they observed it was involved in a collision at Fred Waring and Adonis Dr (approximately
1/4 mile away). Upon their arrival they observed three deceased occupants of one vehicle and the
driver, Ricardo Meza, in the driver seat of the other vehicle. I was notified and responded to a fatal
traffic collision at about 2006 hours and assumed the investigation. All times, speeds and measurements
are approximate. These measurements were obtained with my dual wheel rolatape and a stabon line.

This collision occurred at the intersection of Fred Waring Drive and Adonis Drive within the City of Palm
Desert.
Fred Waring Drive in the area of the collision is a relatively straight, level, major multi-lane, east-west
aligned city road. There are three lanes for travel in each direction. Eastbound lanes have one left turn
pocket lane at Adonis Drive. The turn lane is delineated by a solid painted white line. There is a raised
median with concrete curbs landscaping and decomposed granite separating the east and westbound
lanes. The traffic lanes are constructed of asphalt/concrete. There are raised concrete sidewalks
bordering both the north and south roadway edges. Square concrete gutters separate the
asphalt/concrete lanes from the raised sidewalks on both sides of the road.
Adonis Drive in the area of the collision is a relatively straight, level, north-south road north of Fred
Waring Drive. There are two unmarked lanes for travel in each direction used for commercial business
access. There is a stop sign at the intersection of Fred Waring Drive. The traffic lanes are constructed of
asphalt/concrete. There are raised concrete sidewalks bordering both the west and east roadway edges.

Fred Waring Drive and Adonis Drive intersect at an approximate 90-degree angle with Fred Waring Drive
being a main through street. The intersection is controlled with a stop sign on Adonis Drive. The stop sign
appeared to be in good condition with no obstructions blocking the view of the stop sign. The weather
was clear, and the roadway surface was dry.

Fred Waring Drive is one of the most traveled Streets in the City of Palm Desert. Monterey Avenue is
also one of the most traveled streets in the city of Palm Desert, and is located less than one quarter of a
mile from the area of the collision. During the time of the collision, about 1913 hours, the intersection
of Fred Waring Drive and Adonis Drive was more congested than normal. There were vehicles
commuting from work, heavy tourist traffic on the road, and a nearby church at the intersection to the
collision in session. Hwy 111 also Intersects with Fred Waring Drive which is less than a mile from the
collision. There are numerous restaurants, shops, and a WalMart in the area with pedestrian traffic.
There is a current Engineering and Traffic survey for Fred Waring Drive. The survey was
completed on 01/29/2020 and showed an 85th percentile speed (AKA critical speed or prevailing
speed) on the roadway to be 46 miles per hour which agrees with the posted speed of 45 miles
per hour.
Collision Hfstorv:
Utilizing the City of Palm Desert Traffic Engineering software program (Crossroads), a traffic collision
history report was run for this section of roadway on 01/18/2023. The report detailed all collisions
reported and documented between 01/01/2017 and 12/31/2022. Report limits were set for collisions
that occurred on Fred Waring Drive and Adonis Drive. Based on this report, there were two traffic
collisions in this specific area during a six-year period.

This collision occurred during the night hours. The following data information for Palm Desert, California
(longitude 116.39° W, latitude 33.73° N) on December 28th , 2022, was obtained from the United States
Naval Observatory Astronomical Applications Department: See attachments for sun and moon data.

Party #1 (P-l.Meza) was located in the driver's seat of Vehicle #1 (Honda) when Deputy Taylor arrived on
scene (see supplemental report). P-1 was later identified by his valid California Driver's License. P-1 was
established as the driver of V-1 at the ti me of the collision by his statements, witness statements, P-1 is
the registered owner of V-1 (Honda), and P-1 (Meza) was located in the driver seat after the collision. P-1
was transported to Desert Regional Medical Center by Cal Fire, and Deputy Bloomfield.
Driver License History:
P-1 has a valid driver's license in the state of California. DMV has issued a driver's license card to P-1

On 01/05/2023, Deputy Abrego responded to Fred Waring Drive and Adonis Drive and searched the
surrounding areas for surveillance cameras. Refer to Deputy Abrego's supplemental for further details

speed driving and likelihood of it causing serious injury or death. This admonishment was captured on a
body worn camera.

by Meza.
OPINIONS AND CONCLUSIONS:
Summary:
On Wednesday, 12/28/2022, at about 1913 hours, Deputy Santos and Deputy Taylor were stopped in the
north bound lanes of Monterey Drive at the intersection of Fred Waring Drive at a red light. They
witnessed a black Honda driving west bound on Fred Waring Drive through the intersection of Monterey
Avenue at a estimated speed of over 100 MPH. Traffic conditions at the time was heavier than normal
due to the time of night and the tourist season for the City of Palm Desert. Deputy Santos waited until it
was safe to clear the intersection to attempt a traffic stop on the vehicle. After Deputy Santos cleared
the Intersection he proceeded west bound on Fred Waring Drive and arrived to a traffic collision with the
involved Black Honda and the victims Nissan Sentra. Deputy Santos approached the Nissan and
observed the three deceased victims, and Deputy Tayior approached the black Honda and observed
Meza sitting in the driver's seat.

A (CDR) down load was later completed by Deputy Fransik at Palm Desert Station confirming the Honda
reached a speed of 107 miles per hour 2.5 seconds pre-collision and colliding with the Nissan at 96 miles
per hour. The Nissan was eastbound on Fred Waring Drive attempting to turn left onto Adonis. The high
rate of speed and the night conditions made it nearly impossible for the driver of the Nissan to
determine the closing speed and distance of the approaching Honda.
Partv #2 (P-2. Abel Alvarado) was traveling eastbound Fred Waring Drive in the eastbound left turn
pocket lane. P-2 turned left onto Adonis Drive.was traveling westbound on Fred
Waring Drive in the westbound #1 lane approaching Adonis Drive. P-1 proceeded straight causing a
broadside collision with P-2.
Area of Impact (AO!)
The damage to the involved vehicles and the physical evidence at the collision scene determined the
area of impact. The Area of Impact (Vehicle #1 vs Vehicle #2) was located approximately 18' 6" west of
east curbline prolongation of Adonis Dr and 21' 4" south of the north curbline prolongation of Fred
Waring Dr.
Cause:
Party #1 (P-1. Meza) caused this collision by driving in violation of CVC section 22350(. no person shall
drive a vehicle upon a highway at a speed greater than is reasonable or prudent having due regard for
weather, visibility, the traffic on, and the surface and width of, the highway, and in no event at a speed
which endangers the safety of persons or property. P-1 proceeded straight and approached the
intersection at a high rate of speed colliding into V-2 (Nissan) who was turning left on to Adonis Drive.
This is based on the witness statements and CDR data which displays V-l's reaching a speed of 107 mph
2.5 seconds pre-collision and colliding into V-2 at 96 mph. It is my opinion that P-2 (Alvarado) could not
have anticipated the speed of V-1 (Honda) prior to turning and likely turned when the vehicle was at a
distance where, if traveling at a normal speed, he would have not been involved in a collision.
This cause is based on physical evidence at the collision scene, the damage to the involved vehicles, and

independent statements obtained.
Recommendations:
Based on the physical evidence at the collision scene, independent witness statements obtained, P-l's
driving record, and P-l's prior exhibition of speed conviction and prior admonishment by Deputy Santos;
it is my opinion P-1 knew it was dangerous to drive V-1 at a high rate of speed and could likely result in
death. The evidence at the scene, coupled with the CDR data report; confirmed P-l's speed before and
during the collision to be over double the 45-mph speed limit on Fred Waring Drive. The results of P-1
decisions and actions were a deliberate disregard for human life as he collided with V-2 resulting in the
death of three victims in V-2.
It is my recommendation that the Riverside County District Attorney's Office review this investigation
and consider the following charges for filling against P-1 (Meza):
187 (a) California Penal Code- murder is the unlawful killing of a human being, or a fetus, with malice
aforethought.

BODY WORN CAMERA:
On 02/20/2020,1 was wearing my department issued body wom camera (BWC), video recording
device. The BWC was affixed to the mid chest area of my uniform shirt. Several portions of the
investigation/interviews were recorded on video. The recordings were placed on a disc and were
booked into evidence at the Palm Desert Sheriffs Station. The recordings were also downloaded
and saved to the department server, where they will be kept for 366 days. The body wom camera
footage will reflect the most accurate depiction of this incident.
TRAINING AND EXPERIENCE:
I have been Deputy Sheriff with the Riverside County Sheriffs Depjirtment since 2012 and have
been assigned to the Palm Desert Sheriffs Station as of March 2018. As of May of 2019,1 have
been working with the Riverside County Sheriffs Department Street Racing Enforcement Team
(S.T.R.E.E.T.) county wide as a collateral duty, which is directed at putting a stop to illegal street
racing and takeovers (intersection and private property parking lots).
While being assigned to S.T.R.E.E.T., I have leamed, investigated and worked over 300 hours
combating street racing and takeovers county wide. It has been my experience, those who
participate in illegal street racing activity and win such activity, tend to tum on hazard lights on
after going from point A to point B in efforts to show the other driver and those who tend to

On 02/20/2020, I was working regular uniform patrol in the city of Indian Wells, county of
Riverside, State of California. At approximately 2352 hours, I was parked on a no name street just
east of Yellow Sage on Fred Waring Drive, when I heard two distinctive sounds consistent with
two vehicles accelerating at a high rate of speed (Street Racing/Exhibition of Speed). Shortly
thereafter, I noticed a four door, light colored, sedan traveling at a high rate of speed on the number
#1, eastboimd lane of Fred Waring passing a pickup truck. 1 estimated the speed of the pickup
truck to be traveling at 50 mph. Shortly after passing the pickup truck, the sedan turned its hazard
lights on. As I looked to my right, I noticed another light-colored sedan traveling at a high rate of
speed eastbound, which also passed the same pickup truck that the first sedan drove by. Based on
my training and experience, I estimated the speed of the first sedan to be approximately 85 mph
and the second vehicle involved to be 70 mph.
As I placed my vehicle in drive, I conducted a U-tum and attempted to catch up to both vehicles.
As I completed the U-tum and traveled eastbound Fred Waring, I noticed the first sedan making a
right turn on El Dorado at a high rate of speed against a red steady light. The secondary vehicle
involved in the street racing incident had come to a stop at the intersection due to the same red
light. Due to the driving actions of the first vehicle, I choose to pursue the that vehicle due to the
disregard of other motorists. After accelerating at a high rate of speed, I was able to locate the first
vehicle at Highway 111 and El Dorado. I conducted a traffic stop on the vehicle, Ca. Lie.
#8LDE016 (Silver, 2005 Nissan Altima, 4 Door) at the intersection of Highway 111 and Province
Upon approaching the vehicle, I identified myself as a Deputy Sheriff and advised the driver he
was pulled over for exhibition of speed. The driver of the vehicle was later identified as Ricardo
Meza with his Califomia driver license (CA DL #¥3700215). Initially while speaking with Meza
and requesting for his vehicle documentation, Meza denied racing and stated he was on his way
home from work (Olive Garden in Palm Desert) and did not have an explanation for his speed. I
glanced at the driver license Meza provided me and the driver license stated he lived at 74310
Magnesia Falls Dr. Apt #3., which was approximately 1.5 miles west of the Fred Waring Drive
and Yellow Sage, which was the opposite direction that he was traveling.
After retrieving the required documentation from Meza, I retumed to my unit to conduct my
criminal history of Meza, using law enforcement databases. After completing the history search, I
retumed to Meza's vehicle and asked Meza to step out. Meza stepped out of the vehicle and was
placed under arrest for 23109 (c) CVC (Exhibition of Speed). I later read Meza his Miranda Rights
from my department issued Miranda card, while he sat in the back of my patrol unit with handcuffs.
At the end of every Miranda right that was read, Meza replied "Yes", stating he understood each
of his rights. The following is a summary of the conversation with Meza under Miranda:

22 Based on my investigation, training and experience, and statements made by Meza, Meza was
23 indeed in violation of 23109(c) of the California Vehicle Code, when he chose to partake in illegal
24 street racing with his friend after work. It was clearly shown while interviewing Meza, that he
25 continued to lie with his inconsistent statements which not only contradicted each other and also
28 Meza's vehicle was impounded for 30 days per 23109.2 CVC and his California Driver License
29 was also temporarily suspended for reexamination under DMV Form #FFDL27 due to his reckless
30 and negligent driving habit during this incident. Meza was placed under arrest for 23109(c) CVC
31 and transported to Riverside County, Indio Jail, where he was booked in. A copy of this report will
32 be forwarded to the Riverside Coxmty District Attorney's Office for prosecution against Meza for
33 23109(c) CVC.
10 I then asked Meza why he made a right turn while his fnend continued to go straight. Meza stated
11 he thought his fnend was going to follow him as he was going to use Highway 111 to grab
12 something to eat at In-n-Out. When Meza approached Highway 111 after making a right turn from
13 El Dorado, he then made another right turn going the opposite direction from both In-n-Outs in
14 Indio and La Quinta. Meza told me he was in the wrong and agreed that he agreed after explaining
15 him that it only takes two or more cars traveling the same direction on a public roadway and by
16 him turning on his hazard lights to show off or impress the other person. While explaining the
17 exhibition of speed summary definition above, Meza was shaking his head up and down in
18 agreement while smirking as if it were a funny.
1 Meza stated he was not street racing and was driving normal with his friend in the other vehicle.
2 Meza stated he was not speeding, but agreed he passed the pickup truck that was going the speed
3 limit. Meza then stated he was only going 10 mph over the posted speed limit. When asked what
4 the speed limit was, he stated 55 mph. Meza then stated he was traveling 70 mph and agreed it was
5 not safe to speed. Meza agreed that shortly after passing his friend in the other vehicle, he turned
6 on his hazard lights. When asked why he turned on his hazard lights on he stated, "Because we
7 both have slow cars". Meza then recanted his earlier statement about going home from work and
8 stated he was going to Indio with his friend and then come back home.

EVIDENCE:
On 12/28/2022, I was working as a uniformed Deputy Sheriff assigned to the Palm Desert Special
Enforcement Team. During the incident described below, I was wearing my department issued body worn
camera, which was affixed to my load bearing vest. The body worn camera was activated and the video
was uploaded to evidence.com per department policy.
DETAILS:
On 12/28/2022, about 1911 hours. Deputy Taylor and I were stopped at a red traffic light signal at
Monterey and Fred Waring, within the city of Palm Desert, County of Riverside, State of California. While
stopped at the red light, traffic was moving east and west on Fred Waring with the green light. As the
traffic light for westbound traffic to make a left cycled green, I observed and heard a black sedan driving
westbound on Fred Waring at high rate of speed estimated at 100 mph, based on my training and
experience.
Due to heavy traffic, it took me several seconds to be able to clear the intersection using my emergency
lights which are affixed to my patrol unit. Upon clearing the intersection and made the left turn onto Fred
Waring, in efforts to locate the black sedan traveling at a high rate of speed, which based on time, season
and speed, I believed to have probable cause to believe the driver was driving in a reckless manner and
placing the other motorist at risk.
As we continued to travel westbound Fred Waring, I noticed several other vehicles' brake lights illuminate
and debris on the roadway. As we approached Fred Waring and Adonis, we observed two vehicles, a
silver sedan, and the same black sedan we saw moments involved in a collision. I advised dispatch of our
findings and immediately asked for medial personnel to respond and additional personnel for traffic
control.
As I got closer to the vehicles, I initially noticed two subjects trapped in the silver car. Upon Cal-Fire
arriving they were able to identify a third occupant in the vehicle. At approximately 1925 hours, the
subjects in the silver sedan were pronounced deceased by Cal-Fire and provided dispatch with that
information.
As the driver to the black sedan was being placed in the ambulance, he looked familiar. As I ran his
vehicle through the Department of Motor Vehicle, I was able to identify the driver as the same Ricardo
Meza, I had previously arrested for street racing in 2020. During that arrest, I did advised Meza the
danger of street racing and advised him that the county at time was already dealing with vehicular
homicides for street racing.
Deputy Taylor and I were eventually relieved by the Palm Desert Traffic Team and we left eh scene and
had no further involvement in the incident.

DETAILS:
On 12-28-22, about 1911 hours, while assigned to the Palm Desert Special Enforcement Team, in the City of
Palm Desert, County of Riverside, Deputy Santos #5481 and I were stopped at a red light in the north bound
lanes of Monterey Ave. X Fred Waring Dr. While stopped at the red light, traffic was moving east and west on
Fred Waring Dr. with the green light. As the traffic was moving, 1 observed a black sedan driving west bound
on Fred Waring Dr. traveling through the interseetion at a high rate of speed.
Due to the heavy traffic volume and the speed in which the vehicle was moving. Deputy Santos waited about
fifteen seconds for traffic to come to a stop, then activated the poliee vehicle's red and blue lights to safely
move through the intersection. Once through the intersection. Deputy Santos tumed off the red and blue lights
and we continued west bound on Fred Waring Dr. to attempt to locate the black Sedan.
As we approached Fred Waring Dr. X Adonis we observed two vehicles, a silver sedan, and a blaek sedan, that
appeared to have been in a traffic collision. Dispatch was advised of the traffic collision and our location as
we approached the vehicles to render aid. I approached the black sedan and contaeted the driver, who was
sitting in the driver seat of the black sedan and only identified himself as Rieardo.
As I approaehed the driver, he yelled for help several times and I identified myself. The subject continually
asked for water and to call his mother and tried several times to exit his vehicle. I advised the subject to stay
seated in the vehicle until paramedics could evaluate him. Paramedics arrived and the male was transported to
Desert Reginal Medieal Center (DRMC). At that time, I was assigned to the west perimeter of the traffic
collision to assist with scene security until I was relived at 0024 hours on 12-29-22. Deputy Santos and I left
the seene and had no further involvement in the incident.

PURPOSE;
The purpose of this report is to document my involvement in this case.
PHYSICAL EVIDENCE:
None collected.
BODY-WORN CAMERA:
During this investigation, I was wearing my department-issued Body Worn Camera, an Axon
device, which was attached to the mid-chest area of my Load Bearing Vest. Several portions of
the investigation/interviews were recorded on the audio/video of the camera. I did review portions
of the footage prior to writing this report. The video was uploaded to Evidence.com per department
policy.
DETAILS:
On 12/28/2022, at about 1913 hours, while on uniformed patrol in the city of Palm Desert, county
of Riverside, state of California, I was dispatched to assist in a major injury traffic collision.
I arrived on scene and parked my patrol unit across the westbound lanes of Fred Waring Dr, at San
Luis Dr. I contacted the driver of a white GMC Acadia (Ca license plate 8WSS203), who was
stopped just before the wreckage of the scene. I instructed the driver, to
move his vehicle onto Adonis Dr. as Paramedic's vehicles were entering the scene.
Interview with witness
I spoke with who was the front passenger in vehicle. told me
he was a retired LA County Sheriffs deputy. He witnessed the black Honda pass their vehicle at
a high rate of speed. He was unsure of the actual speed, but he estimated the Honda (VI) travelling
at about 60-70 miles per hour. 1 asked him if his specialty was traffic when he was a deputy, and
he said, "No." said he observed as the vehicle collided into V2 as V2 made the left turn onto
Adonis Dr. did not have any other information.
Interview with witness
I spoke with and the following is a summary of his statement: was driving from
his house to Casa Blanca Restaurant for dirmer with his wife and friends. As they were traveling
west on Fred Waring Dr, passing Monterey Ave, observed a newer black Honda civic
(VI) pass him at a high rate of speed. said, "He was at least going twice our speed." I
asked how fast he was driving, and he said, "About 40(mph)." He then saw V2 turn left
onto Adonis Dr from the turn pocket of eastbound Fred Waring Dr. At that moment
witnessed the impact and saw a cloud of smoke and dust. He then stopped and waited until

5 observed both vehicles travelling westbound on Fred Waring Dr. She believed that one car rear
6 ended another. She appeared confused as to what she observed. She said, "I saw that gray car hit
7 the other car 1 thought." It appears that from her vantage point in the rear of the vehicle she did not
8 witness the seconds before the collision. She did not have any other information.

12 she observed the black car pass their car, "Oh my gosh he was going pretty fast, faster than we
[3 were." She said she observed V2 make a turn but was not sure if the vehicle was making a U-tum

for safekeeping.
*The above items were booked into evidence at the Palm Desert Sheriff Station.
DETAILS:
On Wednesday 12/28/22 at approximately 1945 hours, I received a call out reference a triple fatal
traffic accident that occurred at 1913 hours in the city of Palm Desert County of Riverside, at the
intersection of Fred Waring Drive and Adonis Drive. I responded from my residence and arrived on scene
at 2030 hours.
At the scene I saw a gray Nissan Sentra (CA#7MWK445) partially on the north curb of westbound
Fred Waring Drive just west of Adonis Drive. The vehicle had major damage to the right side of the vehicle
and appeared to have hit a street light pole, which had fallen on the sidewalk. There were two females
deceased in the vehicle along with a deceased male driver. I also saw a black Honda Accord
(CA#8YMW019) further down the street with its front end facing north against the north curb of
westbound Fred Waring Drive. The Honda Accord appeared to have both front airbags deployed along
with the side curtain airbags. The driver of the Honda Accord was not at the scene and was transported
to the hospital prior to my arrival.

Riverside County Sheriff
Deputy Thomas was primary on the call and asked me to start taking photos of the scene due to
Forensic Services unable to reponed to the scene. Forensics were on a homicide in a different part of the
county and were not able to respond. I used a Canon M50 camera and began taking pictures of the scene.
Due to the street light pole being taken out during the accident the lighting was dark near the scene. I
utilized my flashlight and the build in flash on the camera to take photos of the scene.
I used placards A and B to mark and depict the initial impact scuff marks in the roadway. Placard F
marked where a handicap placard was found in the roadway near tire friction marks.
On 12/29/22 at approximately 0015 hours. Riverside County Fire (RCF) was requested back atthe
scene to assist the Corners office to extract the subjects out of the Nissan Sentra. RCF arrived and was
able to extract the driver of the Nissan Sentra, who was identified by his California driver's license
#N4837124 as Abel Duenas Alvarado DOB: 05/18/1953. RCF had to use the jaws of life to cut the roof of
the vehicle to extract the two other females in the vehicle. The second subject to be extracted from the
vehicle was the right front passenger, who's seat was pushed to the left rear seat of the vehicle, due to
the major damage to the right side of the vehicle. The right front passenger door was pushed to the
center of the vehicle up against the driver's right shoulder and face. The right front passenger was
identified by her California identification card #N9093653 as Elena Marron DOB:06/14/1938. Marron
appeared to have numerous broken bones and had major injuries to hertorso, legs and had a deep cut to
her face. The third subject extracted from the vehicle was the right rear passenger, who was identified by
her California Driver's license #N6332747 as Yolanda Alvarado DOB: 02/18/1957. Alvarado had major
injuries to her legs and had her right foot decapitated. Alvarado's right foot was in the vehicle still in the
boot she was previously wearing. I photographed the injuries of all the occupants extracted from the
Nissan Sentra as the coroners performed their preliminary examination at the scene.
On 12/29/22 at 0249 hours, I escorted T&V Tow from the scene to the Palm Desert Sheriff Station
with the black Honda Accord. I arrived with T&V tow at the Palm Desert station at 0259 hours. The Honda
was unloaded off the tow truck and placed into the evidence vehicle bay and secured. I then booked the
evidence I collected atthe scene into evidence atthe Palm Desert Station. Upon weighingthe marijuana
collected from the black Honda Accord, it showed that it was a total amount of .06 grams of marijuana.
The medication collected from the vehicle was placed in a separate bag and entered into evidence for
safekeeping.

On 01/05/23 Deputy Fransik attempted to retrieve the air bag control module from the Nissan
Sentra, but due to the damage to the vehicle the module was destroyed and un-downloadable. I
photographed the module depicting the damage to it and later uploaded the photo onto the Axon server
for evidence.
I also performed a follow up to see if there were any surveillance camera footage from the nearby
businesses in the area but was unable to locate any that displayed the incident. This ended my
involvement in this investigation.

During the course of this investigation I was wearing my department issued Body Worn Camera
(BWC) affixed to the mid-chest area of my uniform shirt. Several portions of the
investigation/interviews were recorded on video. The recordings were downloaded and saved to
the department server.
DETAILS;
On 12/28/2022, at 1914 hours, while assigned uniformed patrol in the City of Rancho Mirage,
County of Riverside, I responded to the intersection of Fred Waring Drive and Adonis Drive, in
the City of Palm Desert, regarding a major traffic collision. I arrived at 1920 hours and was
instructed to ride in the ambulance with one of the involved parties of the traffic collision.
The male was identified as Ricardo Meza and was driving a black Honda Accord bearing CA
license plate 8YMW019. Paramedics advised we were going to Desert Regional Medical Center
for Meza to be treated. While in the ambulance, paramedics asked Meza if he remembered
anything. Meza stated he was coming from Walgreens where he picked up Ibuprofen due to a
work-related injury. While he was driving west of Fred Waring Drive, Meza stated a car pulled
out in front of him. Meza stated he did not have anything to drink and did not take any drugs that
would cause him to be impaired. While in the ambulance, I did not notice any objective signs of
intoxication.
Meza had severe bruising on the stomach area and across his chest caused by the seatbelt when
the collision occurred. Once we arrived at the hospital, medical staff began performing tests to
determine Meza's injuries. After the tests were complete, it was determined Meza suffered a
fractured rib and stomach bruising. I attempted to ask Meza if he remembered anything from the
collision. Meza was unable to remember the collision and had an extremely difficult time
remembering what he did throughout the day. Meza continued to repeat himself saying, "what
happened", and "where am I". Meza eventually was able to tell me parts of the day he remembered.
He stated he went to Walgreens to pick up Ibuprofen, however the line was too long so he left. He
then went and got food and ran other errands throughout the day before returning to Walgreens to
pick up his medication.
Meza could not remember anything before the collision occurred or how it happened. Meza was
unable to tell me what year it was, who the president was, or where he lived during the course my
interaction with him. While at the hospital. Deputy Sanchez arrived and attempted to ask Meza
questions, however he was unable to answer them. Deputy Sanchez provided Meza with a
McNeely warrant to perform a blood draw in order to determine if Meza was under the influence
of an alcoholic beverage or drugs. After his blood was taken, medical staff advised us that Meza
would be getting admitted to the hospital for an unknown amount of time. Deputy Sanchez and 1
left the hospital and had no further contact with Meza.

During event one, prior to any collision, the vehicle was traveling at 107 mph (156.97 ft/s) at -5.0
seconds. The maximum speed of the vehicle was recorded as 108 mph (158.44 ft/s) between -1.5
seconds and -1.0 seconds. With a final speed of 96 mph (140.83 ft/s) at 0 seconds or time of impact.
During event one, between -5.0 seconds and -1.5 seconds, the vehicles Anti-lock Braking System
(ABS) and service brake were indicated as "off. At -1.0 seconds, the service brake was indicated as
"on" with no ABS activity. At -0.5 seconds and 0.0 seconds, the service brake and ABS were indicated
as "on".
During event two, between -5.0 seconds and -2.0 seconds, the vehicles Anti-lock Braking System
(ABS) and service brake were indicated as "off. At -1.5 seconds, the service brake was indicated as
"on" with no ABS activity. At -1.0 seconds, -0.5 seconds and 0.0 seconds, the service brake and ABS
were indicated as "on".
Safety systems indicated in the pre-crash data included a forward collision warning system, collision
mitigation braking system, collision mitigation braking system; forward collision warning, lane
departure warning, road departure mitigation system, and a road departure mitigation; lane
departure warning. All mitigation systems were indicated as "not warning", "not engaged", or "off.
Additional vehicle features included in the pre-crash data included adaptive cruise control, lane
keeping assist, and cruise control. All systems were indicated as "off.

VEHICLE/ SAFETY/ MECHANICAL INSPECTION
PURPOSE:
The purpose of the vehicle / safety / mechanical inspection is to provide information pertaining to the
involved vehicle(s) pre-collision mechanical status and whether or not the vehicle(s) themselves could have
caused or contributed to the cause of the collision. The vehicle inspections were performed at the Palm
Desert Sheriffs Station within the vehicle evidence bay #1. The inspections took place between 0700 hours

I contacted the databases of National Highway Safety Administration (NHTSA) Office of Defeets
Investigations, Recalls, and Technical Service Bulletins (TSBs) via its web site on the internet. The purpose
of the contact was to inquire if there were, or had been, any NHTSA recalls, defect investigations, or TSBs
for a 2021 Honda Accord.
The inquiry revealed no unrepaired recalls.
DAMAGE DESCRIPTION:
The 2021 Honda Accord sustained major damage to the front of the vehicle with the most severe being on
the passenger side front fender and hood. As a result of the collision, the engine compartment and front-end
components were severely damaged. The front windshields was shattered, all other windows were intact.
INSTRUMENTS AND GAUGES:
The 2021 Honda Accord is a digital speedometer was found to be in the "0" MPH position. The headlamp
switch was in the on position; however, due to major damage, I was unable to check any other gauge or
switch within the instrument cluster. During the inspection there was no indication the instrument cluster or
gauges were inoperable prior to the collision.

ENGINE / TRANSMISSION:
The 2021 Honda Accord is equipped with a 4 cylinder, DOHC (Dual overhead cam), 16 valve gasoline
engine and automatic transmission. The radiator had been damaged, and no fluid remained. Due to the
damage the engine oil level could not be inspected. The throttle body was operating within normal range
and was not stuck open. Unless otherwise noted, no errors or faults were found during the
engine/transmission inspection, and it appears all mechanisms were working properly prior to the collision.
STEERING / SUSPENSION:
The 2021 Honda Accord is equipped with power steering. The system was inspected and appeared normal.
The brake master cylinder was damaged, and the contents emptied. The steering wheel was locked and
could not be turned. The rack and pinion portions of the steering components were considerably damaged as
a result of the collision. All components and parts looked intact and appeared normal. Unless otherwise
noted, no errors or faults were found during the steering/suspension inspection, and it appears all
mechanisms were working properly prior to the collision.
BRAKES:
The 2021 Honda Accord is equipped with four-wheel disc brakes and is equipped with ABS. A visual
inspection revealed no signs of pre-collision damage or defects. The front brake pads, front brake rotors,

RESTRAINT SYSTEM / SEATBELTS:
The 2021 Honda Accord is equipped with dual front airbags and lap/shoulder harness seatbelts. The
vehicle's driver side airbag was deployed and appeared to have been working properly during the collision.
The driver's seat belt was found locked in the extracted location, an indication the seatbelt was in use during
the collision. Unless otherwise noted, no errors or faults were found during the restraint system/seatbelt
inspection, and it appeared all mechanisms were working properly prior to the collision.
LIGHTING:
The lighting on the 2021 Honda due to the headlight bulbs being destroyed as a result of the collision and
the filament could not be examined. The headlight lever was in the "on" position. The vehicle's lighting
equipment appeared to have been working properly prior to the collision.
OPINIONS / CONCLUSIONS:
The mechanical examination of the vehicle's steering and suspension systems showed no suspension
modifications, preexisting or otherwise, that may have affected the normal functional ability of the vehicle
upon the highway and/or caused or contributed to the traffic collision.
The meehanical examination of the throttle systems did not reveal any mechanical deficiency, preexisting or
otherwise, that may have affected the normal functional ability of the vehicles upon the highway and/or
caused or contributed to the traffic collision.
The examination of the braking system did not reveal any mechanical deficiency, preexisting or otherwise,
that may have affected the normal functional ability of the vehicles upon the highway and/or caused or
contributed to the traffic collision.
The review of the technical service bulletins derived from the database of NHTSA that were associated with
the examined systems did not reveal any documented deficiencies that would have caused or contributed to
the traffic collision.

of the contact was to inquire if there were, or had been, any NHTSA recalls, defect investigations, or TSBs
for a 2015 Nissan Sentra.
The inquiry revealed no unrepaired recalls.
DAMAGE DESCRIPTION:
The 2015 Nissan Sentra sustained major passenger side damage. As a result of the collision, the engine
compartment and front-end components were severely crushed inward. The front and rear windshield and
all windows were shattered, and the roof was removed by Gal-Fire to extract the occupants.
INSTRUMENTS AND GAUGES:
Unable to inspect the dash due to the damage.

The 2015 Nissan Sentra is equipped with an in-line 4 eyclinder, DOHC (Dual overhead cam), 16 valve
gasoline engine and automatic transmission. Due to the collision the engine was badly damaged. Due to the
damage the engine oil level and condition could not be inspected.
STEERING / SUSPENSION:
The 2015 Nissan Sentra is equipped with power steering. The system could not be inspected due to extreme
damage to the front end.
BRAKES:
The 2015 Nissan Sentra is equipped with front and rear disc brakes with ABS. The system could not be
inspected due to the extreme damage.

I have been assigned to handle this potential claim involving a serious injury and 3 fatalities. The loss occurred
on 12-28-2022, ~7:13 pm, at the intersection of Fred Waring Dr. and Adonis Dr.
Here are my preliminary investigation questions for our silent investigation while we monitor for receipt of
claims:
1. Copy of the TCR including photos and video footage secured at the scene
2. Was major accident response team (MART/MAIT) investigation completed? If so, provide complete
copy of investigation report with any photos/video.
3. complete coroner’s report with toxicology, if applicable
4. Have criminal charges been filed against the responsible motorist?
5. Does the City own and maintain the location? If not, who does? Are there recorded easements? Do
multiple entities share the location?
6. Does the location have a documented history of complaints by residents? If so, what were they and
what was done in response? Please provide copies of any records regarding same.
7. Were any traffic/warrant studies undertaken by the City regarding the location? If so, what were the
results? Were any modifications to the location made? Please provide copies of any reports/records
regarding same.
8. Are signed, as-built plans available for this location. If so, please provide copies of them.

Hi Mariana,
I have been assigned to handle this potential claim involving a serious injury and 3 fatalities. The loss occurred
on 12-28-2022, ~7:13 pm, at the intersection of Fred Waring Dr. and Adonis Dr.
Here are my preliminary investigation questions for our silent investigation while we monitor for receipt of
claims:
1. Copy of the TCR including photos and video footage secured at the scene
2. Was major accident response team (MART/MAIT) investigation completed? If so, provide complete
copy of investigation report with any photos/video.
3. complete coroner’s report with toxicology, if applicable
4. Have criminal charges been filed against the responsible motorist?
5. Does the City own and maintain the location? Yes – it is a public intersection in public right-of-way,
fully within the City of Palm Desert If not, who does? Are there recorded easements? Do multiple
entities share the location?
6. Does the location have a documented history of complaints by residents? If so, what were they and
what was done in response? Please provide copies of any records regarding same. Five service requests
exist in our system. Six screen prints (.jpg format) were saved to W:\Public Works\Adonis Drive As-
Builts.
7. Were any traffic/warrant studies undertaken by the City regarding the location? No records were
found. If so, what were the results? Were any modifications to the location made? Please provide
copies of any reports/records regarding same.
8. Are signed, as-built plans available for this location. If so, please provide copies of them. Mariana, the
as-built plans for the subject location are copied to this temporary folder: W:\Public Works\Adonis
Drive As-Builts.

December 29, 2022 - The Riverside County Sheriff’s Department reports the following. On
December 28, 2022, about 7:13 P.M., deputies responded to a two-vehicle collision at the
intersection of Fred Waring Dr. and Adonis Dr. in the city of Palm Desert. Deputies arrived within
seconds and learned a 2021 Honda Accord was traveling westbound on Fred Waring Dr. at a high
rate of speed when it collided with a 2015 Nissan Sentra that was attempting to make a left turn. The
driver and sole occupant of the Honda, Ricardo Meza, 24-years of Palm Desert, was transported to
an area hospital and is being treated for non-life-threatening injuries. The driver and two occupants
of the Nissan were pronounced deceased at the scene. Toxicology results are pending, so it is
unknown if drugs and/or alcohol played a factor in this collision at this time.
The Palm Desert Sheriff’s Station Traffic Team responded to the scene and assumed the
investigation. The traffic collision investigation required the temporary closure of westbound and
eastbound Fred Waring Dr. between Town Center Way and Monterey Ave. for several hours.
This is an ongoing investigation and anyone with information regarding this incident is encouraged to
call Deputy Claro Sanchez of the Palm Desert Station at 760–836–1600.
