The reporting party at Executive Car Wash reported someone turned off power to the location and observed smoke coming from the electrical box north of the business. The deputy was unable to determine the owner of the property at the time and did not locate any victims / suspects. No report was written due to the fact he could not determine if the damage was recent or intentional. Give complete facts, date, time of day, etc. If applicable provide police report # or case # On May 2, 2024, Claimant, Daniel Casillas went to the property located at 77988 Desert Country Cir., Palm Desert, CA 92211 in search of his "step -daughter" who was believed to be sleeping on the premises. After finding her, they began to gather her belongings. In the process of gathering her belongings, Claimant tripped and fell into an electrical box that was exposed, left open, and contained live wires. As a result, Claimant suffered severe burns throughout his body necessitating emergency medical treatment. Where did DAMAGE or INJURY occur? Use diagram on reverse side of necessary. Give street names and numbers, measurements from landmarks, etc. The injury occurred on the property located at 77988 Desert Country Cir., Palm Desert, CA 92211. What particular ACT or OMISSION do you claim caused the injury or damage. If applicable, provide the name of the City employees involved. Namely, the security gate door was unsecure allowing for access to the area which held electrical box that were left open and exposed, subjecting individuals, including Claimant, to an unreasonable risk of harm. What DAMAGE or INJURY do you claim resulted? Be specific and complete. As a result of the aforementioned negligence, Claimant suffered severe burns throughout his body which necessitated immediate and emergency care and treatment. He also suffered emotional distress including, but not limited to pain, suffering, anxiety, stress, and loss of enjoyment of life. As a result of his injuries, he was forced to take time off work, incurring loss of earnings, and still has not been able to return to his job.
