On April 21, 2021 I contacted you about splitting my head and hurting my neck by hitting a low branch left on trees overhanging the new sidewalks going to the pickleball courts at Freedom Park. You sent me a claim form to be filled out and returned to the city within 6 months of the injury. I am hereby notifying the city through you of my claiming damages for injuries hitting the branch. You indicated you needed a "wet copy". I am enclosing pictures of my claim forms as the "wet copy" was mailed today and should reach you in a few days. In case they get lost or misplaced I want to preserve my claim by sending you pictures of the "wet copy". I had hoped to have this submitted sooner, however the damage to my neck has not resolved itself and due to Covid my health care provider has been slow in allowing me to have face to face appointments with my Doctors to find out what is wrong with my neck. I am in extreme pain and cannont move my neck to the right without lots of pain. I am unable to sleep at night as the pressure of my head on a pillow prevents me from getting a goods night sleep and I often wake up in pain for which I am now taking oxycodine.. I am scheduled tomorrow to talk to a surgeon, again by phone and have to contact the spine and neck clinic for further review and possible surgery... I am a disabled person and suffer much pain due to other health issues. Now I have this to deal with. Please acknowledge the receipt of these pictures of my claim and would you please advise me when you receive. the Wet copies. Thank you ... Walter V. Pisarczyk
