Good morning SS4A Grant Recipients,
As a second reminder if you have not done so, please submit your quarterly reports (progress and Financial) by January 20, 2024 to me. I am attaching two files for your reference.
One is for quarterly reports forms and instructions (I emailed you before) and the other one has form SF-270 when you submit your invoices in Delphi. Please note that these are different
reports, and you need to submit your quarterly progress reports prior to submitting invoices in Delphi.
For submitting invoices, we recommend you check the below items:
1. Verify that you are up to date in submitting quarterly progress and financial reports
2. Include form SF-270 (see attached form with instructions) and make sure items 5 and 8 in the form are sequential to previous requests. Check amounts and totals for mathematical errors
and verify the 20% match by your agency for each requested amount. We reimburse 80% of invoices after approval as per the executed Grant Agreement.
3. Attach supporting documents such as consultant invoices, city or county expenditures that would include name, title, hourly rate, hours worked for the requested period, and total.
Include a brief description of tasks performed for the requested payment period.
4. Form SF-270 must be signed and dated by authorized official
