Patient is a Zl-year-oid female brought to the ED by ambulance after she wasT-boned by
another vehicle going approximately 60 mph. On arrival to the ED the patient Is
complaining of right arm and elbow pain, right foot pain, bilateral knee pain, is GCS 15.
Airbags did deploy, patient was wearing her seatbelt.

GASTROINTESTINAL: soft, NT/ND.
GENITOURINARY/RECrAL: Able to squeeze buttocks. No urethra! blood noted.
MUSCULOSKELETAL/EXT: Right arm deformity at the elbow, tender to palpation at this
area. Bilateral knees and right foot tender to palpation. LUE ROM Intact and nontender to
palpation. No swelling. Compartments soft.
SKIN/INTEGUMENTARY: No jaundice.
BACK; Nontender. No T-spine tenderness. No L-spine tenderness. No swelling or step offs.
NEUROLOGIC: Alert and oriented x 4. Motor strength 5/5 in LUE. Motor strength
decreased in BL LEs secondary to pain. RUE unable to assess motor strength due to pain
and deformity at the elbow. Sensation intact throughout. Speech normal. GCS Total IS
(Eyes 4 (spontaneously), Verbal 5 (oriented), Motor 6 (obeys commands))
PSYCHIATRIC: Normal mood. Appropriate affect.

Post procedure exam: Circulation motor and sensory intact
Immobilization: Fiberglass.
Splint/Strapping (performed/placed by myself) was evaluated for effectiveness of Its
positioning on the affected limb and proper application. Distal neurovascular exam pre-
and post-application of splint was normal.
Patient tolerated: Well
Plan of care was explained to the patient and/or legal guardian. Recommended follow-up
with Orthopedics in 3-5 days.

MEDICAL DECISION MAKING-
Patient is a 21-year-oId female brought to the ED by ambulance after she was T-boned by
another vehicle going approximately 60 mph. On arrival to the ED the patient is
complaining of right arm and elbow pain, right foot pain, bilateral knee pain. Administered
4 mg IV morphine, 4 mg IV Zofran. Left knee x-ray shows ACL avulsion of the tibia! spine
In the left knee. Left knee joint effusion. Right elbow x-ray shows posterior dislocation with
small fracture fragment likely arisen from a posterior condyle of the humerus. Imaging
shows nondisplaced left sacral fracture, and nondisplaced fracture of the left superior and
inferior pubic rami. Right knee, right humerus, right wrist x-rays negative for fracture.
CXR negative for actue cardiopulmonary abnormality. CT of chest abdomen and pelvis
negative, CT of cervical thoracic and lumbar spine negative for acute fracture or
deformity. CBC shows leukocytosis 22, otherwise unremarkable. BMP negative for acute
electrolyte derangement. Liver enzymes elevated, lipase elevated 335. CK elevated 156,

within normal limits.
2100: tiger texted ortho, Dr. Pereira, who recommends CT right elbow after reduction in
the ED, as well as CT of the left knee In order to determine if surgery Is Indicated. States
he will see patient for surgical repair on Monday. Informed trauma team.
Reduction of right elbow performed, see procedure note above. CT right elbow and
CT left knee ordered.
2300: CT read of L knee shows Impacted fracture deformity of the lateral tiblal plateau
with 3 mm depression. Fracture of the tibial spine. Moderate lipohemarthrosis and mild
soft tissue edema. CT of R elbow after reduction shows Reduction of the elbow
dislocation. Small fracture fragments from the lateral epicondyle. Tiger texted Dr. Pereira
CT results, he recommends surgery on Monday with him for knee repair, states he is
unsure about the elbow at this time but will inform patient to follow-up. Informed trauma
surgery about his recommendation.
2350: Trauma surgery admitting for obs and surgery Monday. Informed patient who Is
agreeable to plan.
