Performed by: Resident physician with attending supervision MEDICAL DECISION MAKING- Patient is a 21-year-oId female brought to the ED by ambulance after she was T-boned by another vehicle going approximately 60 mph. On arrival to the ED the patient is complaining of right arm and elbow pain, right foot pain, bilateral knee pain. Administered 4 mg IV morphine, 4 mg IV Zofran. Left knee x-ray shows ACL avulsion of the tibia! spine In the left knee. Left knee joint effusion. Right elbow x-ray shows posterior dislocation with small fracture fragment likely arisen from a posterior condyle of the humerus. Imaging shows nondisplaced left sacral fracture, and nondisplaced fracture of the left superior and inferior pubic rami. Right knee, right humerus, right wrist x-rays negative for fracture. CXR negative for actue cardiopulmonary abnormality. CT of chest abdomen and pelvis negative, CT of cervical thoracic and lumbar spine negative for acute fracture or deformity. CBC shows leukocytosis 22, otherwise unremarkable. BMP negative for acute electrolyte derangement. Liver enzymes elevated, lipase elevated 335.
