HomeMy WebLinkAboutPerformance Table (OWR4) (Revised 011326)CalRecycle Performance Table for the SB 1383 Local Assistance Grant Program Welcome to your Performance Table for CalRecycle's SB 1383 Local Assistance Grant Program (OWR4). This table will verify and track your quantitative data for the Grant Term. Enter data on your project in the appropriate cells. Re-save this file each quarter as "OWR4_[GRANTEE NAME]_PT[corresponding Progess Report #]" (Ex: OWR4_Grantee_PT1) and upload it to the Grants Management System (GMS) in conjunction with your Progress Report on each reporting due date. Please see your Procedures and Requirements document for the specific due dates. If you have any questions, please contact your Grant Manager. Food Recovery Inputs Worksheet In row 3 ‘Edible Food Recovered (tons)’: For edible food recovered (see definition below), input the number of tons recovered and distributed (either as meals, hand out bags, etc.). Do not include hours information regarding volunteers. 1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 Edible Food Recovery and Equipment Notes Refrigeration Equipment Type Volume of System Refrigerant Charge Size Refrigerant Type New Vehicle Type Edible Food Recovered ‘Start Month’: Choose the month and year that the equipment was installed. ‘Equipment Type’: Select the equipment type from the dropdown menu. If more than one type of refrigerator/freezer was purchased, enter each type in the subsequent rows, along with their respective start date. If more rows are needed, contact the CalRecycle Grant Manager. Remember to keep track of the date refrigeration equipment is first used in the Progress Report in the Refrigeration/Equipment Purchased Table under #5 in the Work Completed and Performance Data section. ‘Number of Identical Units’: If only one unit was purchased, you mustenter “1”. If more than one unit was purchased and installed in the same month, enter the number of identicalunits purchased. If more than one of the same type of refrigeration unit is purchased, but they are different sizes or installed in different months, enter the different refrigeration units on separate rows. ‘Volume of System’: Enter the internal volume of the unit or “cabinet volume” in cubic feet. Note: For any residential equipment, the volume is assumed to be 30 cubic feet and no entry is necessary. ‘Refrigerant Charge Size’: No entry necessary. ‘Refrigerant Type’: A default value representing an average of common refrigerant types is assumed. If you would like to specify a refrigerant type, choose the appropriate option from the drop down menu and provide documentation to verify the refrigerant type to your Grant Manager and technical advisor. ‘Start Month’: Choose the month and year that the vehicle began use for the grant project. ‘Vehicle Type’: From the drop down menu, select the vehicle type. See the definitions below for additional infromation or clarification. If more than one type of vehicle was purchased, enter each type in the following rows. If more rows are needed, contact the grant manager and technical advisor. Remember to keep track of the date vehicles are first used in the Progress Report in the Refrigeration/Equipment Purchased Table under #5 in the Work Completed and Performance Data section. ‘Number of Identical Vehicles’: Enter the number of identical vehicles purchased if beginning use in the same month. If only one vehicle was purchased, you must enter “1.” If more than one of the same type of vehicle is purchased, but they begin oppration in different months enter the vehicles on separate rows. In ‘Number of Jobs Provided’ enter the total number of persons hired for that position. If multiple people are performing the same job, they can be combined on one row. In ‘Number of Jobs Provided to Priority Populations’ enter the total number of persons hired for that position from Priority Populations. If multiple people are performing the same job, they can be combined on one row. In ‘Job Permanence’ select an input from the drop down options. In ‘Job Classifications or Trades’ select an input for job classification, trade, or title from the drop down options. In 'Job Education Required' select an input from the drop down options. This is not asking about the education of the person in the position, but the education needed to be hired into the position. In 'Job Experience Required' select an input from the drop down options. This is not asking about the experience of the person in the position, but the experience needed to be hired into the position. For 'Job Training Credentials,' if the project funded job training that results in industry recognized credentials, provide a description of the job training credentials earned as a result of the training (e.g. certification, license, degree). In 'Total Number of Workers that Completed Job Training' enter the number of workers who completed the job training that results in industry recognized credentials. In 'Total Number of Priority Populations Workers that Completed Job Training' enter the number of Priority Populations workers who completed the job training that results in industry recognized credentials. In ‘Project Work Hours’ enter the total number of hours worked on this project during each reporting period. The ‘Total’ project work hours will be calculated automatically. In ‘Priority Populations Project Work Hours’ enter the total number of hours worked by members of Priority Populations on this project during each reporting period. The ‘Total’ project work hours will be calculated automatically. In 'Average Hourly Wage ($)' enter the average hourly wage for the position(s) reported in that row. In 'Average Hourly Wage for Priority Populations ($)' enter the average hourly wage for the Priority Populations position(s) reported in that row. For 'Employer-Paid Health Insurance,' 'Paid Leave,' and 'Retirement Plan,' select "Yes" or "No" from the drop down options. In 'Priority Populations Hiring Strategy,' specify what, if any, targeted hiring strategy was used to hire from Priority Populations. If a project includes the purchase of refrigeration equipment, choose the type of equipment from the drop down menu. A small walk in refrigerator/freezer has a refrigerant charge less than 50 pounds and are used primarily in convenience stores, small grocery stores, pharmacies, and restaurants. A large walk in refrigerator/freezer has a refrigerant charge greater than 50 pounds and are found in supermarkets, large grocery stores, and other retail food establishments. If cell is not applicable, leave blank. If a project includes the purchase of refrigeration equipment, enter the compartment volume of the refrigeration equipment (ft3). If cell is not applicable, leave blank. If a project includes the purchase of refrigeration equipment and the refrigerant charge size is known, enter the refrigerant charge size in pounds. If charge size is unknown, leave the cell blank and defaults will be used. If cell is not applicable, leave blank. If a project includes the purchase of refrigeration equipment and the refrigerant type is known, select the appropriate refrigerant type from the drop down menu. If the refrigerant type is unknown, select "Default Value". If cell is not applicable, leave blank. If a project purchases a vehicle as part of the grant, select the type of vehicle from the drop down menu. If a passenger vehicle or van is purchased, select the appropriate "Van" drop down with the correct vehicle fuel type and whether the vehicle will be refrigerated. If a box truck delivery vehicle is purchased that has a gross vehicle weight rating (GVWR) greater than 14,000 lbs, select the appropriate "Box Truck" drop down with the correct vehicle fuel type and whether the vehicle will be refrigerated. If a heavy duty tractor trailer delivery vehicle with a GWVR greater than 26,000 lbs is purchased, select the appropriate "Heavy Duty Truck" with the correct vehicle fuel type and whether the vehicle will be refrigerated. Enter the pounds of edible food that will be recovered and used to feed people each reporting period (lbs). Do not include the weight of any residual materials that will be landfilled or not used to feed people. Do not include food waste residuals that will be composted or anaerobically digested. If cell is not applicable, leave blank.
