P019, P021, AND C004) Recommendation: By Minute Motion, that the Agency Board: (1) Approve Change Order No. 4 to Contract No. R25590 in the amount of $40,400 for the Phase 2 Civil Improvements at Entrada del Paseo. Executive Summary: Approval of this recommendation will allow the General Contractor to complete the Entrada del Paseo Phase II Civil Improvements. The proposed Change Order addresses minor site modifications, a series of deducts and additions regarding the actual quantities of sidewalk, pavers, and curbing installed, and a revised completion date of May 16, 2008. Attached please find a summary table with itemized costs and credits: Site Modifications: Cost Revised storm drain to accommodate handicap access $8,400 in front of the Henderson Building Actual Material Bid Quantities: Cumulative change $32,000 (see table on Page 2) New Completion Date May 16, 2008 Cumulative Total: $40,400.00 Staff Report Approve Change Order No. 4 to Jacobsson Engineering Construction, Inc. (Contract No. R25590) Page 2 of 3 May 8, 2008 The proposed Change Order will improve the project's over-all safety, accessibility and will close out the project. Change Order No. 4 in the cumulative amount of $40,400 is recommended for approval. Discussion: The original scope of work for the Entrada del Paseo Phase II Civil Improvements is complete with the exception of minor punch list items. The additional scope of work added as part of Change Order 3, which includes the paving of the originally designed turf block access road and the addition of 24 parking spaces is 90% complete. Site Modifications: C.O.R. P021 addresses the conflict between the Phase 2 Civil Improvement plans and the Henderson Community Building Plans. A ribbon gutter location required removal to maintain the handicap parking stalls required 2% grade in front of the Henderson Community Building. This modification resulted in a cost of $8,400. Material Bid Quantities: During the original bid period, contractors were given bid forms with estimates for all civil materials, including but not limited to sidewalk, curb, gutter, pavers and asphalt. C.O.R. P019 address a series of deducts and additions for differences between the estimated quantities provided on the plans and the actual materials utilized by the contractor. All quantities installed have been field verified by the Construction Manager and are justifiable. The series of deducts and additions is summarized below and results in a cumulative increase of $32,000. Staff and the Construction Manager believe this change is justifiable and recommends approval. Based on the Construction Manager's and staff's review, Change Order No. 4 in the amount of $40,400 and a final completion date of May 16, 2008 is recommended for approval to complete construction of all civil improvements. Funds are available in Account No. 850-4358-433-4001. The Contractor shall construct, furnish equipment and materials, and perform all work as necessary or required to complete the Change Order items for a lump sum price agreed upon between the Contractor and the Palm Desert Redevelopment Agency, otherwise referred to as Owner. The drawings scale out to be a lot more along with the actual measurement of the area. The contractor estimates 4200 sf to cover the area infront of La Spiga and near the Henderson Building.
