RESURFACING IN AN AMOUNT NOT -TO -EXCEED $15,096 FOR

Recommendation:
By Minute Motion, that the Agency Board ratify an agreement in an amount not -to -
exceed $15,096.00 with Decorative Concrete & Resurfacing for etching and
sealing the exterior polished concrete at the Henderson Community Building,
located at 72-559 Highway 111, with a non -slip surface. Funds for this service are
available in Account No. 850-4358-433-4001.
Executive Summary:
The Agency's Interim Executive Director authorized staff to contract with Decorative Concrete &
Resurfacing to etch and seal the exterior polished concrete at the Henderson Community
Building, located at 72-559 Highway 111, due to health and safety (slip and fall) concerns. The
agreement includes a five (5) year warranty. Approval of staff's recommendation will ratify a
contractual services agreement in the amount not -to -exceed $15,096.00 with Decorative
Concrete & Resurfacing.
Discussion:
The Notice of Completion for the Henderson Community Building was filed on
October 2, 2008, and the grand opening occurred on October 11, 2008. The Chamber of
Commerce is up and running in their new location. Staff is working to correct a few ADA and
safety concerns, one of which is the slipperiness of the exterior polished concrete.
Bids were received from Del's Flooring, Malibu Marble, and Spektrum Group. Malibu Marble
was the low bidder; however, they provided a sample which was not acceptable to staff. Each
contract bid a slightly different product/process because the concrete was already polished
(finished), but Decorative Concrete & Resurfacing provided an acceptable sample and was the
lowest responsible bidder.

Staff reviewed an on -site sample provided by Decorative Concrete & Resurfacing and found it to
perform very well in wet and dry conditions, and agreed the finished product will preserve the
polished concrete aesthetic. The Interim City Manager, Director of Building and Safety and the
Risk Manager also reviewed the sample and found it to be acceptable. Following the
application of the non -slip surface, staff will have the surface tested and results recorded.
Due to health and safety (slip and fall) concerns, the Interim Executive Director authorized staff
to contract with Decorative Concrete & Resurfacing to expedite the application of the non -slip
surface. Therefore, staff recommends that the Agency ratify the contractual services agreement
with Decorative Concrete & Resurfacing.

SCOPE OF WORK COMPLETED:
1) Etch or profile approx. 6,400 sq feet of polished concrete
with the primary goal being slip and fall safety,and also
preserving the polished concrete aesthetic.
2) Apply a primer coat of Mirathane WB
3) Apply a second coat with #60 silica sand in a satin/matte
finish for non slip purposes ... can add more sand if desireable.
* Sq Footage is 485 on South end,3,144 sq ft West end,2,815 on
Front/East end of Bldg. Total Sq Footage = 6,424
* All material spec sheets and MSDS information provided
Payment Terms: 10%---- 60%---- 30%

* Decorative Concrete Recommends and will provide periodic
touch up on an "as needed basis" i.e. (main traffic areas) after
warranty expiration at a reduced fee of .75c per sq ft...
DC&R/BKS Inc. guarantees against loss of adhesion for a period of 3 years on exterior work,10 years Interior/Garage
Flooring.and 10 years on Interior floor staining (under normal wear & tear) and provided there is ongoing wax maintenance.
In the event of adhesion failure, DC & R /BKS,inc agrees to repair the effected area free of charge. The following limitations apply
• This warrantv does not warranty against anv cracks. cracking or crack repair of anv nature whatsoever. Including those
created by Earthattakes. Earth shifting or settling ... only against loss of adhesion of materials annlied.
