Application Overview
The following pre-qualification questionnaire provides the information necessary to determine whether you qualify to be placed on the interest list for the City of Palm Desert's for-sale
housing programs. This does not guarantee or imply that you will be assisted with the purchase of a home.
If any listed applicant or household member is represented by a real estate agent or broker at any time during the application or purchase process, it must be disclosed. The Housing
Department may correspond with your agent with regard to a property as a courtesy, but will assume no responsibility for payment of any fees to your agent.
It is important that you notify the Housing Department of any changes in your income, household size or composition, or any other information requested on the application while you are
on the interest list. This helps the Housing Department determine whether you qualify for the next available home. If the Housing Department does not have your most current information
when a suitable home becomes available, you may be notified that you do not qualify because your information was not updated.

Buyers will need to execute an affordability covenant for up to 45 years to restrict affordability.
A down payment is required in order to purchase a home in a for-sale development. The down payment must be identified at the time of prequalification.
Minimum stated amount: $4,000, but amount varies by development. Applicants and co-applicants must agree to maintain
the property as their principal residence during the entire affordable period. I am aware that the Palm Desert
Housing Authority does not warrant the condition or value of the property. Right to Financial Privacy Act of 1978,
12 U.S.C. 3401 et seq. You authorize the Authority to have access to financial records held by financial institutions in connection with the consideration or administration of assistance
to you. Financial records involving your loan and loan application will be available to the Authority without further notice or authorization, but will not be disclosed or released
by the Authority to another government agency or department without your consent except as required or permitted by law.
Section VII. Applicants Certification
The information provided in this questionnaire and its attachments is true and correct as of the date set forth opposite my signature. Any intentional or negligent misrepresentation
of the information contained in this questionnaire and its attachments may result in civil liability, including monetary damages, to any person who may suffer loss due to reliance upon
any misrepresentation made in this questionnaire and its attachments. The Authority and its agents, brokers, insurers, servicers, successors, and assigns may rely on the information
contained in this questionnaire, and I am obligated to amend or supplement the information provided if any material fact changes.
Transmission of this questionnaire as an electronic record containing my electronic signature, or by facsimile containing a facsimile of my signature, shall be as effective, enforceable,
and valid as if a paper version of this questionnaire were delivered containing my original written signature.

Please provide copies of the following documents. These documents become part of the questionnaire package and must be received in order to review your qualifications for the for-sale
programs.
State of California Driver's License or Identity Card for all applicants and household occupants.
Social Security Card for all applicants and household occupants (optional). Although a Social Security card is not required, if identity cannot be confirmed, the application may be rejected.
Income documentation for all applicants and household occupants, as applicable:
Most recent 2 payroll stubs
Social Security (SSA-1099 Benefit Statement, or direct deposit verification via bank statements)
Pension (last 2 check stubs, monthly statement, or end-of-year statement)
Unemployment or Disability (check stub, award statement, and/or end-of-year statement)
Alimony/Child Support (divorce decree, separation agreement, court documents, or verification by deposits shown on bank account statements)
If self-employed, provide 3 years of business tax returns, 6 most recent business bank statements, and year-to-date profit and loss statements.
Federal income tax returns for the last 3 years. Include the most recent year as a printed copy and include all pages, schedules, attachments, W-2s, 1098s, and 1099s. Previous years
may be digital. If exempt from filing federal income tax returns, sign a Declaration of Taxpayer Non-Filing of Federal Income Tax Return Status form, if applicable.
Bank account statements for the last 3 months for all financial institutions, including all pages.

Under the Housing Financial Discrimination Act of 1977, it is unlawful to discriminate in the provision of or the availability of financial assistance because of consideration of:
Trends, characteristics, or conditions in the neighborhood or geographic area surrounding a housing accommodation, unless the financial institution can demonstrate in the particular

Information Sheet: Desert Rose
Desert Rose is an affordable housing development consisting of 161 single-family homes located two blocks east of Cook Street on Fred Waring Drive.
The community includes three- and four-bedroom attached and detached homes ranging from approximately 1,100 to 1,300 square feet. Amenities include common areas, an adult pool and spa,
a children's pool, volleyball and basketball courts, and a YMCA childcare center.
Since sell-out in 1998, demand to live in Desert Rose has remained high. Occasionally, an owner decides to sell and a home becomes available.
The Palm Desert Housing Authority maintains an interest list of people interested in acquiring a home at Desert Rose if one becomes available. Homes are available to very low, low, and
moderate income families. Prospective buyers are selected from the list according to income and family size appropriate for the available home. The Authority provides limited financial
assistance for eligible households who qualify for a primary loan with a conventional lender.
Restrictions have been placed on the homes to ensure affordability for up to 45 years, and buyers must meet certain minimum qualifications at the time of purchase.

Community Information Sheet: Falcon Crest
Falcon Crest is a for-sale community consisting of 93 single-family homes located in the heart of Palm Desert near recreation facilities, shopping, and services.
The community was developed by the former Palm Desert Redevelopment Agency as part of its affordable housing program and made available to households whose income was at least $40,000
annually. Three- and four-bedroom floor plans were designed with today's households in mind.
The project has a Tuscan theme with drought-tolerant front yard landscaping. Homes include ample living spaces with nine-foot ceilings and abundant natural light. Energy-efficient features
include R-30 attic insulation, R-19 insulated 2x6 walls, double-paned low-e windows, and Energy Star air conditioning.
The Palm Desert Housing Authority maintains an interest list of people interested in acquiring a home at Falcon Crest if one becomes available. Homes are available to very low, low,
