ALL QUESTIONS MUST BE ANSWERED, ANSWERS MUST ADDRESS THE SPECIFIC QUESTION, AND REQUIRED DOCUMENTATION MUST BE ATTACHED. INCOMPLETE APPLICATIONS WILL NOT BE REVIEWED FOR FUNDING CONSIDERATION. If additional space is required to answer questions included in this application, you may attach additional sheets. Answers continued on attached sheets must be clearly numbered and in sequence with the application. Copies of this application are available via email upon request. Funds may be available for approved agencies and/or organizations upon HUD approval after July 1, 2022. Reimbursement for qualifying expenditures will be retroactive to July 1, 2022. You are encouraged to submit your application well before the deadline to allow ample time for review of completeness and accuracy. QUALIFYING CRITERIA In order to qualify for CDBG funding, all eligible projects must meet one of the following criteria: 1. The activity predominantly benefits low and moderate -income persons (more than 51 % of persons benefiting — generally 80% of the area median income). 2. The activity will eliminate slums or blight. 3. The project will meet a need having a particular urgency. The condition of urgency must be of recent origin, generally being developed or critical less than 18 months preceding the application for assistance. Original applications with all required attachments must be submitted to the address listed below no later than 3:00 pm by April 8, 2022. CITY OF PALM DESERT CDBG — FY2022/2023 Attention: Maria Gallegos, CDBG Consultant, Michael Baker International Ph:951-961-9068 Email: maria.gallegos(i�.mbakerintl.com The City of Palm Desert has been designated by the U.S. Department of Housing and Urban Development (HUD) as a "Metropolitan Center', and as such is a Direct Entitlement City. Each year the City applies for federal Community Development Block Grant (CDBG) funds from HUD as a Metropolitan Center to meet the housing, economic, and community development needs of the community. The funds are distributed to various non-profit organizations and jurisdictions for projects and programs that meet the identified needs of the community. The guideline for the HUD approved funding breakdown is, 1) 20 percent for CDBG Program Administration, 2) 65 percent for capital improvement, rehabilitation, housing, planning/studies/strategic planning, etc., and 3) 15 percent for public service programs. Note: Public service programs should focus on activities such as job training and employment services, health care and substance abuse services, childcare, crime prevention, counseling, medical, homeless services, or other programs designed to provide a service to 51 % or more low-income clientele, which must be verified through self -certification forms completed by participants of the program/organization. This application form is used by potential grant applicants who are submitting an application for CDBG funding consideration. Funds awarded must primarily benefit residents of the City of Palm Desert. That evidence must include documentation of at least one community meeting or special Board meeting open to the public this year at which the specific project or program was discussed, and an opportunity was given for citizen input. Describe the methods used to obtain or will be used to obtain citizen involvement and attach appropriate documentation. The applicant shall comply with all Federal and City policies and requirements affecting the CDBG program. 3. The federal assistance made available through the CDBG program funding is not being utilized to substantially reduce the prior levels of local financial support for community development activities. 4. FALSE CLAIMS ACT - The False Claims Act (31 U.S.C. 5 3729-3733, also called the Lincoln Law) is an American federal law that allows people who are not affiliated with the government to file actions against businesses, organizations or persons claiming fraud against the government. Under the False Claims Act those who knowingly submit, or cause another person or entity to submit, false claims payment of government funds are liable for three times the government's damages plus civil penalties of $5,500 to $11,000 per false claim. The False Claims Act explicitly excludes tax fraud. Section 3729(e) states that the Act "does not apply to claims, records, or statements made under the Internal Revenue Code."
